PRIVACY NOTICE FOR CALIFORNIA RESIDENTS
This PRIVACY NOTICE IS APPLICABLE TO CALIFORNIA RESIDENTS (“consumers” or “you”) ONLYand supplements the information in MEDVera’s general Privacy Statement. We adopt this notice to comply with the California Consumer Privacy Act of 2018 (“CCPA”), the California Privacy Rights Act ("CPRA"), and other California privacy laws. Any terms defined in the CCPA or CPRA have the same meaning when used in this notice.

Sources of Personal Information
We obtain the categories of Personal Information listed above from the following categories of sources:
Directly from you: For example, from documents or intake forms that you provide to us related to the services for which you engage us.
Indirectly from you: For example, through information we collect from you in the course of providing services and from your activity on our website.
From third parties: From vendors and partners that interact with us in connection with the services we perform.
Personal Information does NOT include:
Information lawfully made available from federal, state, or local government records.
De-identified or aggregate consumer information (as defined in the CCPA).

2. Sensitive Personal Information
Sensitive Personal Information includes information that reveals crucial personal identifiers. Unless the information is publicly available, it includes:
Your Social Security, driver’s license, or passport number.
Your account log-in, financial account, debit card, or credit card number in combination with any required security or access code, password, or credentials allowing access to the account.
Contents of your mail, email, and text messages unless MEDVera Holdings LLC is the intended recipient of your communication.
Biometric information processed for the purpose of uniquely identifying you.
Personal information collected and analyzed concerning your health, sex life, or sexual orientation.
3. Use of Personal Information
We may use or disclose the Personal Information we collect for one or more of the following business purposes:
To fulfill or meet the reason for which the information is provided.
To provide you with information, products, healthcare management solutions, or services.
To provide you with email alerts, portal updates, and other notices concerning products, services, events, or news that may be of interest to you.
To carry out our obligations and enforce our rights arising from any contracts entered into between you and us, including for billing and collections.
To improve our website and present its contents securely to you.
For testing, research, analysis, and product development to enhance our healthcare platform.
As reasonably necessary and proportionate to protect the rights, property, or safety of us, our clients, or others.

We will not collect additional categories of Personal Information or use the Personal Information we collected for materially different, unrelated, or incompatible purposes without providing you prior notice.
4. Sharing and Selling Personal Information
Disclosures for a Business Purpose: In the preceding twelve (12) months, we have disclosed the listed categories of personal information to our affiliates, service providers, vendors, and authorized third parties for essential business operations.
Sales of Personal Information: We do not sell your Personal Information to anyone.
5. Your Rights and Choices
The CCPA provides California residents with specific rights regarding their Personal Information.
Access to Specific Information and Data Portability Rights
You have the right to request that we disclose certain information to you about our collection and use of your Personal Information collected on or after January 1, 2024. Once we receive and verify your request, we will disclose:

The categories of sources for that information.
Our business or commercial purpose for collecting it.
The categories of third parties with whom we share that Personal Information.
The specific pieces of Personal Information we collected about you (data portability request).
Correction Requests
You have the right to request that we correct inaccurate personal information that we maintain about you. After verification, we will evaluate and update our records accordingly.

However, please note that we only use Sensitive Personal Information to perform services reasonably expected by our users (such as managing your healthcare profile and processing payments securely).
Deletion Request Rights
You have the right to request that we delete your Personal Information from our records, and direct our Service Providers to do the same.
Deletion Exceptions: We may deny your deletion request if retaining the information is necessary for us or our Service Providers to complete transactions, fulfill the terms of a written warranty, detect security incidents, debug products, comply with legal obligations (such as medical record retention laws), or make internal uses that reasonably align with consumer expectations.
6. Exercising Your Rights

Only you, or a person registered with the California Secretary of State that you authorize to act on your behalf, may make a verifiable consumer request related to your Personal Information. You may only make a verifiable consumer request for access or data portability twice within a 12-month period.
Verification Requirements
The request must provide sufficient information that allows us to reasonably verify your identity (and confirmation that the personal information relates to you) and describe your request with enough detail to allow us to properly evaluate and respond to it. We cannot respond to your request or provide Personal Information if we cannot verify your identity or authority.
Response Timing and Format
We endeavor to respond to a verifiable consumer request within forty-five (45) days of its receipt. If we require more time (up to 90 days total), we will inform you of the reason and extension period in writing.
We do not charge a fee to process or respond to your request unless it is excessive, repetitive, or manifestly unfounded. If we determine that the request warrants a fee, we will provide you with a cost estimate before completing your request.
7. Non-Discrimination
We will not discriminate against you for exercising any of your CCPA/CPRA rights. Unless permitted by law, we will not deny you services, charge you different rates, or provide a different level or quality of care.
8. Changes to Our Privacy Notice
We reserve the right to amend this Privacy Notice at our discretion and at any time. When we make changes, we will notify you by email or through a prominent notice on our website homepage.
