Important Safety Information

Cialis® (Tadalafil)

Prescription Medication

Cialis (tadalafil) is an FDA-approved prescription medication for erectile dysfunction (ED) and benign prostatic hyperplasia (BPH) in adult men. Its extended half-life provides effectiveness for up to 36 hours, offering greater flexibility compared to shorter-duration PDE5 inhibitors. Cialis is available in as-needed doses (10mg or 20mg) and low-dose daily formulations (2.5mg or 5mg). A valid prescription from a licensed provider is required.

Emergency: If you experience chest pain, severe shortness of breath, a prolonged painful erection (over 4 hours), or sudden vision/hearing loss — call 911 or go to your nearest emergency room immediately.

Who Should Not Use Cialis® (Tadalafil)

  • Women or children
  • Men taking nitrate medications in any form (oral, sublingual, transdermal, or recreational)
  • Men taking riociguat
  • Individuals with a known hypersensitivity to tadalafil
  • Men with severe hepatic impairment
  • Men for whom sexual activity poses a significant cardiovascular risk

Warnings & Precautions

Nitrate Contraindication

Tadalafil combined with nitrates can cause a sudden and severe drop in blood pressure. This combination is strictly contraindicated and potentially life-threatening.

Duration of Action

Due to its 36-hour duration, tadalafil's effects and potential interactions extend beyond a single sexual encounter. This should be considered when managing other medications during this window.

Priapism

Seek emergency medical attention for any erection lasting more than 4 hours. Prolonged erections may result in permanent damage if untreated.

BPH & Cardiovascular Effects

In patients using Cialis for BPH, clinically significant hypotension may occur, particularly when combined with alpha-adrenergic antagonists. Appropriate dosing intervals should be observed.

Renal & Hepatic Considerations

Dose adjustment is required for patients with moderate or severe renal impairment. Use in severe hepatic impairment is not recommended.

Common Side Effects

This is not a complete list. Contact your provider if you experience any concerning symptoms.

  • Headache
  • Back pain or myalgia (more common with tadalafil than other PDE5 inhibitors)
  • Facial flushing
  • Nasal congestion
  • Indigestion
  • Limb pain
  • Dizziness

Drug Interactions

Always inform your provider of all medications, supplements, and substances you are currently taking.

  • Nitrates — strictly contraindicated (all forms)
  • Riociguat — contraindicated
  • Alpha-blockers — dose separation recommended to reduce hypotension risk
  • Antihypertensives — additive blood pressure lowering possible
  • CYP3A4 inhibitors (ketoconazole, erythromycin, ritonavir) — increase tadalafil exposure
  • Rifampin and other CYP3A4 inducers — may significantly reduce tadalafil effectiveness

When to Contact Your Provider

  • You experience sudden vision or hearing loss
  • An erection lasts more than 4 hours
  • You develop chest pain, severe dizziness, or syncope
  • You take nitrate medications and have been prescribed Cialis in error
  • You want to discuss switching between as-needed and daily dosing regimens
